Why Open Source is Vital for Global Health

A mother in rural Guatemala walks two hours to a clinic where her child's vaccination records are kept in a paper ledger. A community health center in Mississippi cannot afford to upgrade from a discontinued EHR system, so staff manually re-enter data into spreadsheets. A public hospital in Kenya uses three different systems that cannot share patient information, forcing doctors to rely on patients to remember their own medication lists. These are not edge cases. They represent the reality of healthcare technology for most of the world's population.

Open source software does not solve every problem in healthcare. But it removes one of the most persistent barriers: the cost and control of the technology itself.

The True Cost of Proprietary EHR Systems

The financial burden of commercial EHR software extends far beyond the sticker price. A mid-size primary care practice in the United States can expect to pay:

  • $500 to $800 per provider per month for cloud-hosted EHR software
  • $30,000 to $100,000 for implementation, data migration, and training
  • $10,000 to $50,000 annually for interfaces, customizations, and mandatory upgrades
  • Ongoing per-claim fees for integrated billing and clearinghouse services

For a five-provider practice, the total cost of ownership over five years can exceed $500,000. For a safety-net clinic operating on slim margins, these numbers are disqualifying.

But the financial cost is only part of the problem. Vendor lock-in creates dependencies that are difficult to escape:

  • Patient data is stored in proprietary formats that are expensive to export
  • Custom workflows and templates built over years are not portable to another system
  • Contracts include automatic renewal clauses and steep early termination penalties
  • API access for data exchange is often limited or charged as a premium add-on

When a vendor raises prices, degrades support, or is acquired by a private equity firm focused on cost-cutting, practices have no good options. They pay more, accept less, or undertake the enormous disruption of switching systems. In every scenario, patients bear the consequences through higher costs, longer wait times, or degraded care experiences.

Why Communities Need to Own Their Health Infrastructure

Healthcare technology is infrastructure, as essential as roads, water systems, and electrical grids. When a community depends on a single private company for its health data systems, it surrenders control over a critical public resource.

Consider what happens when a proprietary EHR vendor:

  • Discontinues a product line. Practices scramble to migrate data under tight deadlines, often losing historical records in the process.
  • Gets acquired. The new owner may raise prices, change terms, or shift development priorities away from the features that small practices depend on.
  • Experiences a prolonged outage. With no access to the source code, the organization cannot diagnose the problem, implement a workaround, or switch to a backup system. They simply wait.
  • Restricts data access. Despite the 21st Century Cures Act's information blocking provisions, some vendors make data portability technically difficult through proprietary formats, limited export tools, or restrictive API rate limits.

Open source software returns control to the community. When the source code is available under a strong copyleft license, no single entity can take it away, restrict access to it, or hold it hostage.

The AGPL-3.0 License: Guaranteeing Freedom

Ciyex is licensed under the GNU Affero General Public License v3.0 (AGPL-3.0). We chose this license deliberately because it provides the strongest protections for the community.

The AGPL-3.0 guarantees four freedoms:

  1. Freedom to use the software for any purpose, including commercial use
  2. Freedom to study the source code and understand exactly how it works
  3. Freedom to modify the software to meet local needs, add features, or fix bugs
  4. Freedom to distribute modified versions, as long as they remain under the same license

The "Affero" clause adds a critical protection for cloud-hosted software. Under a standard GPL license, a company could take Ciyex, modify it, run it as a hosted service, and never share their changes. The AGPL closes this loophole: if you run a modified version of Ciyex as a network service, you must make your modified source code available to the users of that service.

This ensures that improvements to Ciyex benefit everyone, not just the company that made them.

How Open Source Serves Patients Directly

The connection between open source software and patient outcomes may not be immediately obvious. But the impact is direct and measurable:

Lower costs mean more access. When a free clinic does not pay $50,000 per year for software licenses, those dollars go to hiring a nurse practitioner, purchasing medications, or extending clinic hours. Every dollar saved on software is a dollar spent on care.

Transparency builds trust. Patients increasingly want to know how their data is stored, who can access it, and what happens to it. Open source software provides verifiable answers. The security model, the access controls, the data storage mechanisms are all visible in the source code. No "trust us" required.

Local adaptation saves lives. A clinic in rural India needs different clinical workflows than a family practice in Ohio. Open source software can be adapted to local languages, local clinical protocols, local regulatory requirements, and local infrastructure constraints without waiting for a vendor to prioritize those markets.

No abandonment risk. When a proprietary vendor goes out of business or discontinues a product, its users are stranded. Open source software survives its original developers. As long as someone has the source code, the software can be maintained, forked, and improved.

The Global Health Perspective

The World Health Organization estimates that over half the world's population lacks access to essential health services. In many low- and middle-income countries, the barriers include not just physician shortages and supply chain challenges, but the absence of basic health information systems.

Proprietary EHR vendors have little incentive to serve these markets. The revenue potential is too low to justify the investment in localization, training, and support. As a result, the communities with the greatest need for health IT have the least access to it.

Open source platforms change this equation:

  • Governments can deploy and customize the software without licensing negotiations or procurement delays
  • Universities can use the platform for medical education and health informatics training
  • NGOs like Partners in Health, Doctors Without Borders, and the Clinton Health Access Initiative can standardize on a common platform across their global operations
  • Local developers can build expertise by contributing to the codebase, creating a sustainable local health IT workforce

OpenMRS and DHIS2 have demonstrated this model successfully in lower-income settings. Ciyex extends it to provide a modern, FHIR-native platform that meets the interoperability requirements of both developed and developing healthcare systems.

What "Free" Really Means

Free in the context of open source means freedom, not just zero cost. Organizations can:

  • Self-host on their own servers with complete control over their data
  • Use managed hosting from third-party providers who offer Ciyex as a service (those providers must share their modifications under the AGPL)
  • Customize the software for their specific clinical workflows, regulatory requirements, and patient populations
  • Contribute improvements back to the community, benefiting every other deployment

There are legitimate costs associated with running any software: hosting, support, training, customization. Ciyex does not eliminate these costs. What it eliminates is the artificial cost of the software itself, the licensing fees that add no clinical value and serve only to generate revenue for vendors.

Building a Sustainable Open Source Organization

As a 501(c)(3) nonprofit, Ciyex is structured for long-term sustainability without compromising on open source principles. Our funding model includes:

  • Foundation grants from organizations focused on healthcare access and health equity
  • Government funding through programs that support health IT infrastructure for underserved communities
  • Corporate sponsorships from organizations that benefit from a healthy open source healthcare ecosystem
  • Paid support and consulting for organizations that need implementation assistance, custom development, or training

This model ensures that Ciyex remains free for those who need it most, while generating the revenue necessary to sustain full-time development, security maintenance, and community support.

Healthcare technology should serve the public good. Open source makes that possible. The AGPL-3.0 license makes it permanent.
